If your incident is life-threatening or a serious crime is occurring, please call 000 immediately. Where the incident being reported involves an allegation against an individual it is important that the Incident Liaison Officer be the only person to commence an investigation and will be in contact with you. If your incident is classified at a level of Major or Higher and your incident relates to a Person/Injury/Accident please call (08) 6240 7798 If you would like to lodge a complaint, please use the Complaint Form If your incident is actually related to an Asset, please use the Asset Incident Form Please refer to notification matrix for the classification of your incident.
